EyeSpy 20/20 Vision Screening Planning Tool

Follow these simple steps to plan your EyeSpy 20/20 vision screenings. Use this tool to plan up to 10 schools. (If your district has more than 10 schools, divide your screening project into phases with each phase consisting of 10 schools or less.)

STEP 1 - DETERMINE ESTIMATED TIME AND NUMBER OF PROCTORS

Use the interactive table below to determine approximate screening time and proctors needed to conduct your vision screenings.
For each school, enter the following information:
* School Name
* Number of Proctors / Volunteers
* Number of Computers / Screening Lanes (operating simultaneously)
* Number of Students in each Grade you will screen
* Number of Special Education Students (regardless of grade level)

STEP 2 - DETERMINE YOUR SCREENING MODEL

     [     ] School Nurse Vision Screens One on One (small schools)
     [     ] Vision Screening Organization (i.e. Lions, Rotary, etc.) Perform Screenings for each School


STEP 3 - IDENTIFY AND ACCOMMODATE SPECIAL SITUATIONS

     [     ] 2 or more schools located on the same campus / location
     [     ] Schools with an all special needs population
     [     ] Schools with no nurse on-site
     [     ] Schools with part-time nurse / job sharing
     [     ] Specific vision screening requirements for your State


STEP 4 - COORDINATE WITH YOUR I.T. DEPARTMENT

     [     ] Identify & secure correct number of Windows-based computers for the vision screening pod or each school location
     [     ] Whitelist the EyeSpy 20/20 Program & Database domains
     [     ] Upload student rosters
     [     ] Download, install, & calibrate EyeSpy 20/20 software on computers


STEP 5 - ATTEND TRAINING

     [     ] Attend workshop training and / or webinar training


STEP 6 - ESTABLISH ROLLOUT PHASES

     [     ] Establish the "go live" date
     [     ] Establish the number of phases and number of schools per phase
     [     ] For each phase, identify vision screening dates and times for each school
     [     ] Determine where vision screenings will be conducted (library, computer lab, cafeteria, classroom, multi-purpose room, etc.)
     [     ] Allocate and schedule proctors & resources accordingly OR coordinate with a screening organization
